Unsatisfactory Condition Report (UCR)
The unsatisfactory condition report (UCR) program is the FAA-wide system for employees to report occupational safety and health (OSH) hazards. The online reporting and recordkeeping system is hosted by the Safety Management Information System (SMIS) on the FAA intranet. It provides all agency employees with direct means for advising the agency of an existing unsatisfactory condition and allows the union to track them.
Employees are required to discuss unsatisfactory conditions, other than those initiated to address OSH hazards, with their supervisor before initiating the UCR.
A written UCR should be initiated even in cases where the hazard is immediately eliminated to ensure that we are able to learn from situations identified by others. This data is also important in identifying trends that may require additional attention by union reps during workplace inspections, areas where additional employee safety training may be appropriate, or modifications are needed for equipment design and installation or maintenance procedures.

T-SAP (ATO)
The Technical Operations Safety Action Program (T-SAP) provides Air Traffic Organization (ATO) Tech Ops employees with a means to voluntarily and confidentially report aviation safety concerns observed or experienced while acting in their official capacity as an employee.
T-SAP is a non-punitive, voluntary and cooperative approach to identifying and reporting potential aviation safety hazards in the National Airspace System (NAS).
Submitting a T-SAP Report
Technical Operations employees can submit a T-SAP report online. Eligible submitters must complete a one-time registration process before logging in. For assistance registering, logging in or other technical support call the T-SAP hotline at 1-877-360-6961 and choose option 1.
It is important to submit event-based reports within one working day of the event. If necessary, an extension of three (3) working days to file a report will be granted to each submitter who provides a verbal notification via the T-SAP hotline.
Once submitted, that report is prepared for the next phase of the program, which is a thorough analysis of the reported aviation safety concern by the Event Review Committee (ERC). The ERC is comprised of one representative from ATO, one representative from PASS, and one representative from the Air Traffic Safety Oversight Service (AOV) along with designated alternates for each position. Each ERC representative is empowered to make independent decisions within the context of the T-SAP Memorandum of Agreement.
The ERC reviews and analyzes reports submitted; identifies actual or potential aviation safety problems from the information contained in the report; and proposes solutions for those problems. Decisions made by the T-SAP ERC are by consensus in the best interest of aviation safety. Any corrective action recommended by the ERC for a report accepted under T-SAP must be completed to the satisfaction of all members of the ERC.
T-SAP: Open to All
T-SAP has been deployed on a national basis and is available to all ATO employees assigned to Technical Operations: PASS bargaining unit employees, non-bargaining unit employees or management. Each employee who submits a report will receive individual feedback.
T-SAP trainers have provided face-to-face training nationwide on how to submit a report. Face-to-face training also continues at the Mike Monroney Aeronautical Center for new hires and future field training is being planned and will be conducted as needed.

Everyone who is considered to be T-SAP eligible can file a report whether or not they have received the mandatory training.

PASS Letterhead
All business conducted with the agency and other PASS members should be done in a professional manner, and any letters directed to the agency should be on PASS letterhead. Download letterhead
PASS Business Cards
Business cards are available to all contract administration representatives and should include your name, title, at least one phone number and passmember.net email address. You can also include additional information such as alternate phone numbers, a fax number or address. If you wish to order business cards, please contact Communications Director Liz Doherty.
Rep Directory
The contract representative directory is available for each region on both the public and member-only websites, under Regions. It is a valuable resource if you have a question and your regional vice president is not immediately available.
Union Office
The CBA allows for the use of Air Traffic Organization and Aviation Safety facilities for a PASS office. Identify a location that is available for a union office and make an official request. If the request is denied, contact your regional vice president.
Meetings and Telecons
Chapter Meetings
The difference between chapter and contract administration representatives is that chapter business is for internal union activities. Be sure to stay in contact with chapter leaders so you are aware of meetings and events. They are your opportunity to get feedback from the chapter and its members on representational issues.
Contract Rep Telecons
Be sure to regularly participate on contract rep telecons which are an excellent way to keep all reps informed of local activity.
